ABSTRACT

Introduction: The cecum is the first part of the large bowel. Cecectomy is a sufficient treatment for some patients, avoiding overtreatment by ileocolic resection. Purpose:The goal of this study was to review a surgeon's experience with laparoscopic cecectomy and provide a technical video demonstration of this uncommon operation. Methods: A retrospective chart review was conducted of all consecutive patients treated with laparoscopic cecectomy over a 16-year period. All operations were performed using a 3-trocar technique. The cecum was transected with 1 to 2 firings of a 60 millimeters linear stapler, preserving the ileocecal valve and ascending colon. Results: 19 patients were identified including 12 females (63.2%). Median age was 42 years (range 16-84). Indication for surgery included appendiceal pathology in 12 patients (63.2%) and cecal abnormality in 7 (38.9%). There was no conversion to open surgery. Median intraoperative blood loss was 25 ml (range 0-150 ml) and no patient received a blood transfusion. No intraoperative or postoperative complication was noted. The median length stay was 1 day (range 0-6). Readmission rate was 0%. Final appendiceal histopathology revealed acute/chronic appendicitis in 5 patients, mucinous cystadenoma in 4 patients. Cecal histopathology revealed adenoma in 4 patients. Median follow-up was 16 months (range 4-53). Conclusions: Laparoscopic cecectomy is a sufficient treatment for some patients with benign conditions of the appendix and cecum. It carries minimal morbidity. It should be considered as an alternative to segmental bowel resection in a select group of patients. (AU)

ABSTRACT

OBJECTIVES@#To study the clinical efficacy of ultrasound-guided endoscopic retrograde appendicitis therapy in children with appendix-related chronic abdominal pain.@*METHODS@#A retrospective analysis was performed on the medical data of 30 children with the chief complaint of chronic abdominal pain who were admitted from August 2019 to May 2021. All the children were found to have inflammation of the appendix or intracavitary stool and fecalith by ultrasound and underwent ultrasound-guided endoscopic retrograde appendicitis therapy. The medical data for analysis included clinical manifestations, endoscopic findings, white blood cell count, neutrophil percentage, length of hospital stay, and cure rate.@*RESULTS@#Among the 30 children with chronic abdominal pain, there were 13 boys (43%) and 17 girls (57%), with a mean age of (9±3) years (range 3-15 years) at diagnosis. The median duration of the disease was 12 months, and the median length of hospital stay was 3 days. The children had a median white blood cell count of 6.7×109/L and a neutrophil percentage of 50%±13%. Fecalith and a large amount of feces were flushed out of the appendix cavity for 21 children (70%) during surgery. The follow-up rate was 97% (29/30), and the median follow-up time was 11 months (range 5-26 months). Of the 29 children, abdominal pain completely disappeared in 27 children (93%).@*CONCLUSIONS@#Ultrasound-guided endoscopic retrograde appendicitis therapy is effective in children with chronic abdominal pain caused by feces or fecalith in the appendix cavity.

ABSTRACT

INTRODUCTION@#This study aimed to investigate the ultrasonographic features of paediatric acute appendicitis and incorporate them into a scoring algorithm that will quantify the risk of complications and the strength of recommendation for surgical intervention.@*METHODS@#179 patients with suspected appendicitis who had undergone ultrasonographic examination were included in this study. Based on their medical evaluation and post-surgical histopathological results, patients were categorised into confirmed appendicitis (n = 101) and non-appendicitis (n = 78) groups.@*RESULTS@#In the appendicitis group, the appendix was visualised in 66 (65.3%) patients. In cases where the appendix was not visualised, we looked out for secondary inflammatory signs, which were present in 32 (31.7%) patients. Using stepwise logistic regression, Blumberg's sign, free fluid or collection, hyperaemia, non-compressible appendix and an appendix diameter > 7 mm were found to be significant predictive factors for appendicitis. A new scoring system called POPs was developed, combining inflammatory predictors and ultrasonography findings, with an area under the receiver operating characteristic curve of 0.958 (95% confidence interval 0.929-0.986).@*CONCLUSION@#The newly developed POPs-based diagnosis scheme proved a promising alternative to existing scoring systems such as the Alvarado score. Although further calibration would be beneficial, the proposed scoring scheme is simple and easy to understand, memorise and apply in the emergency room.

ABSTRACT

Abstract Tumours of the appendix are rare and tend to be diagnosed incidentally, in cases of acute appendicitis. For some authors, appendiceal neuroendocrine tumours (ANETs) are the most frequent neoplasm of the appendix, observed in 0.3% to 0.9% of cases acute appendicitis. The present is a unicentric retrospective study conducted between January 2005 and March 2017. Out of a total of 3,007 surgeries for appendiceal pathologies performed in the adult population at the hospital where the present study was conducted, there were 70 (2.33%) malignant cases, 20 (28.6%) of which were ANETs. The patients had a median age of 44 years (range: 18 to 85 years), and were predominantly women (there were 1.9 times more women than men). In 16 cases (80%), a simple appendicectomy was performed (1 patient was submitted to a right hemicolectomy later). The cases of ANETs had a good prognosis in our series: 85% of the patients are either alive today or were alive after 5 years of follow-up. Despite the fact that ANETs are described as the most frequent tumor of the appendix, this was not confirmed in our series, in which they only represented 28.6% of the cases; adenocarcinoma was the most frequent tumor (65.7%) among our sample.


Resumo Tumores do apêndice são raros, e tendem a ser diagnosticados de forma incidental, em casos de apendicite aguda. Para alguns autores, tumores neuroendócrinos do apêndice (TNEAs) são as neoplasias mais frequentes do apêndice, e são observados em 0,3 a 0,9% de todos os casos agudos de apendicite. Este é um estudo monocêntrico e retrospectivo realizado entre janeiro de 2005 e março de 2017. Entre umtotal de 3.007 cirurgias para patologias do apêndice realizadas na população adulta no hospital em que o estudo foi conduzido, houve 70 (2,33%) casos de malignidade, 20 (28,6%) dos quais eram TNEAs. Os pacientes tinham uma idade média de 44 anos (gama: 18-85 anos), e eram predominantemente mulheres (havia 1,9 vezes mais mulheres do que homens). Em 16 casos (80%), realizou-se uma simples apendicectomia (1 paciente foi submetido a uma hemicolectomia direita mais tarde). Os casos de TNEAs tiveram um bom prognóstico em nossa série: 85% dos pacientes estão vivos hoje, ou estavam após um seguimento de 5 anos. Apesar de os TNEAs serem descritos como os tumores mais frequentes do apêndice, isso não foi confirmado nesta série, na qual eles representaram apenas 28,6% dos casos; adenocarcinoma foi o tumor mais frequente (65,7%) emnossa amostra.

Introducción: El mucocele es una dilatación apendicular por secreción mucosa secundaria a la obstrucción de la luz, su incidencia estimada es de 0,2-0,3 por ciento de todas las apendicectomías realizadas y el 8-10 por ciento de todos los tumores apendiculares. Objetivo: Presentar un caso inusual de cirugía de urgencia, con dolor abdominal en fosa ilíaca derecha, sugestivo de apendicitis aguda operado en el servicio de urgencia. Caso clínico: Paciente femenina de 32 años de edad, que consulta por dolor abdominal en fosa ilíaca derecha asociado a náuseas, con dolor a la palpación en dicha zona, defensa muscular y dolor a la descompresión brusca del abdomen, fue intervenida quirúrgicamente con el diagnóstico presuntivo apendicitis aguda. Se encuentra durante videolaparoscopia al apéndice cecal aumentado de tamaño, sin compromiso de su base, por lo cual se decide realizar laparotomía y apendicectomía convencional. La paciente fue egresada a las 48 horas de operada con evolución favorable y pendiente de resultado anatomopatológico, el cual informó mucocele apendicular. Conclusiones: El mucocele apendicular es una entidad poco frecuente, que predomina en el sexo femenino y edades entre 50 y 60 años. El tratamiento de elección es el quirúrgico(AU)


Introduction: The mucocele is the abnormal mucous accumulation inside the light of the appendix. Objective: To present an unusual case of surgery of urgency with abdominal pain and suspicion of appendicitis operated in emergency. Clinical case: Female patient of 32 years old that consults for abdominal pain in the right iliac fossa, accompanied by nausea. Physical examination we find pain to the palpation in the right iliac fossa and peritoneal reaction being operated with suspicion of appendicitis, during intervention we find the appendix augmented of volume and apendicectomy was made. The histopathological study informed a apendicular mucocele. The patient was discharged after 48 hours without further complications. Conclusions: Apendicular mucocele is very uncommon disease that predominates in female sex and persons between fifty and sixty years old. Treatment is surgical(AU)

ABSTRACT

Abstract Introduction Crohn's Disease is a chronic and idiopathic inflammatory process with transmural invasion that can affect the entire gastrointestinal tract. The etiopathogenesis of this pathology is not fully understood and studies have been carried out to understand the influence of different kind of factors on its development, including appendectomy. This monograph aims to address the possible existence of a link between appendectomy and Crohn's Disease, and the possible causes and clinical consequences of this association. Methods This monograph was based on the research of original scientific articles in MEDLINE database via PubMed, restricted to articles in Portuguese and English during the period between 1991 and 2017. Results Appendectomy seems positively associated with the development of Crohn's Disease, especially in the first years of surgery, regardless of whether or not there is inflammation of the appendix. In fact, the appendix plays important roles in gastrointestinal integrity, acting in the development of an adequate immune response, maintaining and regulating the intestinal flora. Conclusion The appendix is important for intestinal homeostasis, preventing the development of certain pathologies. Its resection, regardless of whether or not there is an inflammation after surgery, increases the risk of Crohn's Disease and worsens the prognosis of this pathology, so appendectomy should be avoided in the absence of appendicitis.


Resumo Introdução A Doença de Crohn é um processo inflamatório crónico e idiopático com atingimento transmural que pode afetar todo o trato gastrointestinal. A etiopatogenia desta patologia não está completamente esclarecida pelo que se tem vindo a realizar estudos para perceber a influência de diferentes fatores no seu desenvolvimento, entre os quais a apendicectomia. Esta monografia visa abordar a existência de uma possível relação entre apendicectomia e Doença de Crohn e as possíveis causas e consequências clínicas desta associação. Métodos Esta monografia foi elaborada com base em artigos científicos originais pesquisados na base de dados MEDLINE via PubMed, com restrição a artigos em português e inglês com limite temporal de 1991 a 2017. Resultados A apendicectomia parece associar-se positivamente ao desenvolvimento da Doença de Crohn, principalmente nos primeiros anos após a cirurgia, independentemente de haver ou não inflamação do apêndice. De facto, o apêndice desempenha importantes funções na integridade gastrointestinal, com influência no desenvolvimento de uma resposta imunológica adequada e na manutenção e regulação da flora intestinal. Conclusão O apêndice é importante na homeostasia intestinal, prevenido o desenvolvimento de determinadas patologias. A sua ressecção, independentemente do facto de haver ou não inflamação aquando da cirurgia, aumenta o risco de Doença de Crohn e piora o prognóstico desta patologia, pelo que a apendicectomia deve ser evitada na ausência da doença.

INTRODUCTION: Femoral hernias have a high risk of strangulation or obstruction, and their acute presentations are associated with a high risk of mortality, occurring in up to 60% of cases with complications. The presence of the appendix within the femoral sac has been described in a maximum of 1% of cases, which has been called Garengeot hernia, but only 0.08% with concomitant appendicitis. CASE REPORT: Woman 56 years old, cholecystectomized, with no other relevant history, 5 months of evolution, right femoral region volume increase, consultation for 24 hours of evolution of habitual pain exacerbation, CT scan showing complicated hernia, with appendix inside of the hernial sac. The right inguinal approach is performed, the cecal appendix is the only content within the hernial sac, with signs of acute appendicitis, appendectomy and hernia repair with Prolene mesh. Diagnosis is confirmed with biopsy. DISCUSSION AND CONCLUSION: Acute appendicitis within De Garengeot's hernia is extremely rare and often difficult to diagnose. The diagnosis of a femoral hernia appendicitis is often obscured by nonspecific clinical findings that indicate a complicated hernia. Computed tomography may be useful to definitively diagnose acute appendicitis contained within a femoral hernia. It is important to always consider De Garengeot hernia as a differential diagnosis in patients with inguinal mass of acute pain, so as not to delay its detection and management.

ABSTRACT The incidence of tumors in the appendix has increased over the years, and they are mainly found in the anatomical and pathological examination of appendices operated due to acute appendicitis. The annual incidence of neuroendocrine tumors of the appendix, also called carcinoid tumors, is 0.15-0.16 per 100,000 people. In absolute terms, the incidence of these tumors has increased in the last decade by 70-133%. Appendiceal carcinoid tumors occur more often in women, and are found in 0.3-0.9% of the appendices removed in appendectomies. They appear in the subepithelial neuroendocrine cells and have an indolent course, with the symptoms being indistinguishable from an acute appendicitis. There are two classifications, one presented by the European Neuroendocrine Tumor Society and the other by the American Joint Committee on Cancer. Both classifications use tumor size as a predictor of tumor burden. The classification used by European Neuroendocrine Tumor Society also uses the invasion of the mesoappendix to select the best surgical treatment. However, these classifications require the inclusion of more criteria to define the selection of surgical treatment of tumors between 1 and 2 cm. Thus, along with the size of the tumor and the invasion of the mesoappendix, other factors such as vascular invasion, ki67 index, mitotic index and tumor location should be considered at the time of classification, for a better selection of the treatment and prognostic evaluation.


RESUMO A incidência de tumores no apêndice tem aumentado ao longo dos anos, principalmente encontrados no exame anatomopatológico dos apêndices operados por apendicite aguda. A incidência anual de tumores neuroendócrinos do apêndice, também designados por tumores carcinoides é de 0,15 a 0,16 por 100.000 pessoas. Em termos absolutos, a incidência destes tumores tem aumentado na última década em 70% a 133%. Os tumores carcinoides do apêndice ocorrem mais em mulheres e são encontrados em 0,3%‒0,9% dos apêndices removidos em apendicectomias. Têm origem nas células neuroendócrinas subepiteliais e apresentam um curso indolente, sendo os sintomas indistinguíveis de uma apendicite aguda. Existem duas classificações, a apresentada pela ENETS (European Neuroendocrin Tumor Society) e da AJCC (American Joint Committee on Cancer). Ambas as classificações utilizam o tamanho do tumor como preditor de carga tumoral. A classificação utilizada pela ENETS recorre ainda à invasão do mesoapêndice para selecionar o melhor tratamento cirúrgico. Contudo, estas classificações necessitam incluir mais critérios para definir a escolha do tratamento cirúrgico de tumores entre 1‒2 cm. Assim, para além do tamanho do tumor e da invasão do mesoapêndice, outros fatores como a invasão vascular, o ki67, o índice mitótico e a localização do tumor devem ser considerados no momento da classificação, para uma melhor seleção do tratamento e avaliação prognóstica.

ABSTRACT

RESUMEN El epónimo hernia De Garengeot queda reservado para describir la presencia del apéndice cecal dentro de un saco herniario crural. Es infrecuente el hallazgo del apéndice dentro del canal femoral, situación que se presenta en el 0.9 % de las hernias femorales. La presencia de apendicitis dentro del canal femoral es una rareza, representando del 0.13 a 0.8% de todos los casos de apendicitis aguda. Se presenta un caso, se describe el cuadro clínico y el tratamiento quirúrgico (AU).


ABSTRACT The eponym De Garengeot´s hernia is reserved to describe the presence of the cecal appendix inside a crural hernia sac. It is infrequent to find the appendix inside the femoral canal, location presented in 0.9 % of femoral hernias. The presence of appendicitis inside the femoral canal is a rarity representing from 0.13 to 0.8 % of all the cases of acute appendicitis. A case is presented, and the clinical characteristics and the surgical treatment are described (AU).

Objetivo: El objetivo de este estudio es analizar los resultados de nuestro centro en cuanto al cierre del muñón apendicular en la apendicectomía laparoscópica, mediante el empleo de endograpadora. Material y método: Análisis retrospectivo de las apendicectomías laparoscópicas con sección de la base apendicular con endograpadora, llevadas a cabo entre enero de 2013 y marzo de 2015, recogiendo variables demográficas, clínicas y de técnica quirúrgica. Resultados: Se incluyeron en el análisis 238 pacientes. Se registraron complicaciones en 41 pacientes, con una tasa de reingreso del 5% y un 2,5% de pacientes reintervenidos. La estancia media posquirúrgica fue de 3,64 días. Discusión y conclusiones: En el cierre del muñón apendicular, la endograpadora se presenta como una técnica adecuada en cuanto a estancia postoperatoria y complicaciones posquirúrgicas. A pesar de las ventajas de su uso, debe valorarse en apendicectomías no complicadas el empleo de otras técnicas como endoloops.


Objective: The objective of this study is to analyse the results in our centre as regards the closure of the appendix stump in laparoscopic appendectomy using an endo-stapler. Material and methods: A retrospective analysis, collecting the demographic, clinical, and surgical technique variables, was conducted on laparoscopic appendectomies with section of the appendix base with an endo-stapler performed between January 2013 and March 2015. Results: A total of 239 patients were included in the analysis. Complications were recorded in 41 patients, with a re-admission rate of 5%, and 2.5% of patients received further surgery. The mean post-surgical stay was 3.64 days. Discussion and conclusions: The endo-stapler is a suitable technique in the closure of the appendix stump, as regards post-surgical hospital stay and complications. Despite the advantages of its use, the employment of other techniques, such as endo-loops, should be evaluated in non-complicated appendectomies.

ABSTRACT

La incidencia de la hernia de Amyand es muy baja. Siempre es diagnosticada en el transoperatorio, resulta casi imposible de realizar durante el pre-operatorio. Paciente femenina de 75 años de edad, con antecedentes de hernia inguinal derecha recidivante. Comenzó con dolor en epigastrio y en región inguinal derecha, además de náuseas y vómitos. Se identificó cicatriz quirúrgica y aumento de volumen en región inguinal derecha. Se decidió intervenir quirúrgicamente con el diagnóstico clínico de hernia inguinal derecha recidivante encarcelada. Durante el acto operatorio se identificó apendicitis aguda supurada como contenido del saco herniario inguinal. Se procedió a la realización de la apendicetomía y reparación de la hernia, en el mismo tiempo quirúrgico, y colocó la malla de polipropilenoe. Se aplicó antibióticos de amplio espectro. La paciente evolucionó de forma favorable. La biopsia confirmó el diagnóstico. El siguiente caso se presentó, pues a pesar de ser conocido, su incidencia es muy baja, por lo que existen dificultades para su diagnóstico (AU).


The incidence of Amyand's hernia is very low. It is always diagnosed in the trans-operatory period, being almost impossible during the pre-operatory period. This is the case of a female patient, aged 75 years, with antecedents of recidivist right inguinal hernia. It began with pain in epigastrium and in the right inguinal region in addition to nausea and vomits. Surgical scar and volume increase in the right inguinal region were identified. It was decided to make a surgery with the clinical diagnosis of incarcerated recidivist right inguinal hernia. During the surgery an acute suppurated appendicitis was identified as the content of the inguinal hernial sac. The appendectomy was carried out and hernia was repaired in the same surgical time; a polypropylene mesh was placed. Broad spectrum antibiotics were applied. The evolution of the patient was satisfactory. The biopsy confirmed the diagnosis. The case was presented because despite of being known, its incidence is very low, so there are difficulties for diagnosing it (AU).

ABSTRACT

Objetivo: Dar a conocer una rara presentación de una patología frecuente. Caso clínico: Presentamos el caso de un paciente de 27 años de edad con clínica de apendicitis aguda con antecedentes de haberse realizado 9 años atrás una apendicectomía, encontrándose un apéndice inflamado, en posición retrocecal, ascendente y subseroso. En la segunda apendicectomía se encontró un nuevo apéndice sano en la unión de las 3 tenias. El estudio histológico posterior confirmó que ambas piezas quirúrgicas eran apéndices, uno inflamado y el otro normal.


Aim: To report a rare presentation of a frequent pathology. Case report: We report a patient of 27 years old with symptoms of acute appendicitis and medical records of an appendectomyy 9 years before where surgical findings were an inflamed appendix retrocecal ascendant and subserosa. In our procedure, we found a new one at the junction of the three tapeworms. Histological examination later confirmed that both surgical specimens were appendices, one swollen and the other normal.

ABSTRACT

Fundamento: La peritonitis secundaria a apendicitis aguda perforada por esquistosomiasis es una entidad muy rara, pocos casos han sido descritos en la literatura; esta parasitosis proviene de zonas endémicas y con tratamiento antihelmíntico remite hasta su curación, evitando su propagación y complicaciones, que son mortales en estadíos avanzados. Objetivo: Describir el caso de un paciente con peritonitis secundaria a apendicitis aguda perforada por esquistosomiasis. Presentación de caso: Hombre filipino, saludable, de 42 años, con dolor abdominal difuso e intenso de cinco días de evolución, acompañado de vómitos y fiebre e intensa reacción peritoneal, constatada al examen físico, en la tomografía computarizada contrastada de abdomen se visualizó el apéndice engrosado así como colección de líquido intraabdominal. Se realizó laparotomía exploradora, apendicectomía y lavado de la cavidad abdominal. El estudio histopatológico informó, apendicitis perforada con presencia de huevos de Schistosoma en las paredes del apéndice Conclusiones: El paciente evolucionó favorablemente y sin complicaciones posoperatorias, se egresó a los diez días después de terminado el tratamiento antibiótico y antihelmíntico con prazicuantel.


Background: Secondary peritonitis and acute appendicitis percuss by schistosomiasis is a rare entity , few cases have been described in literature , this parasitoids comes from endemic zones and with an antihelmintic treatment letting up to its cure , avoiding its propagation and complications , which are mortal in advanced stages. Objectives: to describe the case of a patient with Secondary peritonitis and acute appendicitis percuss by schistosomiasis. Case report: A filipine healthy man, 42 years old, with an abdominal diffuse and intense pain of 5 days of evolution, together with vomits, fever and intense peritoneal reaction, palpable in the physical exam, in the contrasted computer tomography of abdomen was observed an enlarged appendix as well as collection of intraabdominal liquid. An explore laparotomy, an appendectomy, a wash of the abdominal cavity, and the histopathology reported percuss appendicitis with presence of schitosoma eggs on the walls of the appendix. Conclusions: The patient evoluted favorably and without post operatory complications, egress 10 days after having finished the antibiotic and the antihelmintic treatment with praziquantel.

ABSTRACT

La presencia de un saco herniario inguinal ocupado por un apéndice cecal inflamado es conocida en la literatura como hernia de Amyand. La descripción data del 1735 por Claudius Amyand, quien realiza la primera apendicetomía satisfactoria en un niño de 11 años. Su incidencia en la edad pediátrica es rara, y usualmente es diagnosticada en el curso de la evaluación de una hernia inguinal complicada. Se presenta un paciente de 7 meses de vida que recibió atención médica en nuestra institución, y se realizó, además, una revisión de la literatura en relación con esta entidad.


The presence of an inguinal hernial sac occupied by an inflamed cecal appendix is known in scientific literature as Amyand's hernia. The description dates back to 1735 when Claudius Amyand performed the first satisfactory appendicectomy in a 11 years-old boy. The incidence of this disease is rare at pediatric ages and is usually diagnosed in the course of evaluation of a complicated inguinal hernia. This is a 7 months-old patient who was attended to and treated in our institution and additionally, a literature review on this entity was made.

Torsion of appendices ep loicae is an uncommon differential diagnosis of a patient presenting with acute abdominal pain and its presentation as a Ogilvie’s syndrome or colonic ileus is even rare.The diagnosis requires a keen suspicion on the part of the attending surgeon. Preoperative computrized tomography ( CT) may help in the diagnosis but the final diagnosis is established only on the operating table . Adding this pathology to the existing list of the causes of colonic ileus /Ogilvies syndrome may give us a good insight into the understanding of the condition&improvise our management. This is a case report of 60 year old lady who was referred to us with nonspecific abdominal pain , distention of abdomen and vomiting from orthopaedics department , where she was being planned for surgery for intertrochanteric fracture femur. All our investigations were equivocal and suggested a Ogilvie’s syndrome like picture .Atrial of neostigmine therapy was also given after excluding physical obstruction and the patient did not respond and the patient was operated upon and intraoperative finding of torsed epiploic appendage was found as the cause of her symptoms. The patient made a good post operative recovery following surgical excision of torsed epiploic appendages. This case should give us another dimension of thought in the management of such cases and help improvise our management.

ABSTRACT

Cecal appendix invagination was first described by Mc Kidd in 1858, however, due to its low frequency and nonspecific presentation, it is usually a radiological or intraoperative finding. During surgery, the therapeutic approach may vary from an appendectomy to a right hemicolectomy. The selected technique will depend of the suspected diagnosis, which will undoubtedly also influenced by surgeon experience. We present a case of a male patient aged 57 years old, with the intraoperative finding of appendiceal intussusception. The laparoscopic dissection of the cecum revealed an inflammatory appendix, soft to palpation with graspers. A large section of the appendix base with linear stapler was made. The final biopsy and surgical specimen showed an appendiceal inflammation without atypia.


La invaginación del apéndice cecal fue descrita por primera vez en 1858, sin embargo, debido a su baja frecuencia y presentación inespecífica suele constituir un hallazgo radiológico o intraoperatorio. En este último caso la conducta terapéutica puede variar desde la apendicectomía hasta la hemicolectomía, ya que se debe plantear a la neoplasia cecal dentro de los diagnósticos diferenciales y, en virtud de ello, la decisión operatoria puede obligar a la hemicolectomía derecha. Esta decisión se verá influida también por la experiencia del cirujano. Se presenta el caso de un paciente de sexo masculino de 57 años de edad con el hallazgo intraoperatorio de invaginación apendicular. Se realizó una disección amplia del ciego por vía laparoscópica y a la palpación con las pinzas el apéndice y el ciego impresionaban de consistencia blanda, no tumoral. Debido a estos hallazgos se decidió realizar la apendicectomía realizando una sección amplia de la base apendicular con stapler lineal cortante. La biopsia definitiva de la pieza operatoria informó un proceso inflamatorio apendicular sin hallazgos sugerentes de atipías.

Background: The frequency of appendicular presence of Enterobius vermicularis varies from 0.2 to 41.8 percent. It is generally a pathological finding. The luminal obstruction by the parasite may unveil an acute appendicitis. Aim: To study the frequency of Enterobius vermicularis presence in surgical pieces of appendectomies. Material and Methods: Analysis of surgical pieces of appendectomies processed at the pathology laboratory of a general hospital, between 1993 and 2010. Results: In 21.038 surgical pieces, the presence of Enterobius vermicularis was found in 322 (1.5 percent). Fifty nine percent were women and 71 percent were younger than 18 years. Acute inflammation was confirmed in 71 percent. A history of previous abdominal pain was present in 40 and 70 percent of patients with and without acute inflammation in the surgical piece. Conclusions: In this series, the frequency of Enterobius vermicularis presence in surgical pieces of appendectomy was 1.5 percent.


Introducción: La frecuencia de Enterobius vermicularis (EB) apendicular varía entre 0,2-41,8 por ciento, siendo generalmente su diagnóstico un hallazgo al estudio histopatológico. La obstrución luminal puede desencadenar un cólico apendicular o evolucionar a una apendicitis aguda, siendo por tanto una causa frecuente de apendicectomía. El objetivo del estudio es determinar la prevalencia de EB en piezas quirúrgicas de apendicectomía junto con describir características clínico-morfológicas. Material y Método: Cohorte retrospectiva de pacientes con oxiuriasis apendicular diagnosticados en el Hospital Hernán Henríquez Aravena entre 1993-2010. Se estudiaron variables clínico-morfológicas, utilizándose estadística descriptiva y analítica, aplicando Chi-cuadrado y test exacto de Fisher para variables categóricas y T-student para variables continuas con un IC de 95 por ciento. Resultados: En 21.038 apendicectomías se verificaron EB en 322 casos (1,53 por ciento). El 59 por ciento eran mujeres y 71 por ciento menor de 18 años. Inflamación aguda fue confirmada en 71 por ciento. En casos con inflamación aguda la temperatura rectal y recuento leucocitario promedio fue 380 C y 12.975 mm³ respectivamente, mientras que en casos sin inflamación 37,80 C y 10.984 mm³ (p = N.S). En el grupo apendicectomizado por sospecha de apendicitis el 40 por ciento tenía historia de dolor abdominal previo, mientras que en aquellos sin inflamación aguda asociada el 72 por ciento (p = 0,005). Conclusiones: La prevalencia de EB apendicular fue 1,53 por ciento, afectando fundamentalmente a niños-jóvenes del sexo femenino. La oxiuriasis apendicular representa una causa de dolor abdominal interpretado como apendicitis aguda, sin asociarse a inflamación en el examen anatomopatológico. La historia de dolor abdominal previo resultó una variable útil para discriminar cólico apendicular de apendicitis aguda.
